November is American Diabetes Month and with the number of Americans diagnosed with diabetes expected to triple by 2050, it is more important than ever to schedule a visit with your local SIHF Healthcare professional. Our trained providers are dedicated to making sure you fully understand the seriousness of diabetes and even offer an educational program called SIHF Diabetes Care & Management Program, which is designed to help patients manage their uncontrolled Diabetes through access to care, specialists, and education.
What is diabetes?
Diabetes is a chronic, metabolic disease characterized by elevated levels of blood glucose (or blood sugar), which leads over time to serious damage to the heart, blood vessels, eyes, kidneys and nerves.
There are an estimated 1.5 million new diabetes cases diagnosed each year and this disease remains the 7th leading cause of death in the United States.

Who Qualifies?
Patients with hospitalizations and ER visits related to uncontrolled Diabetes
Patients who have Medicaid (WellCare or Meridian) insurance
Patients who reside in the Metro-East area
